About the role

An exciting opportunity exists for qualified and experienced Support Coordinators to be based in our brand new Eight Mile Plains office.

The  Support Coordinators will provide Support Coordination to participants assisting them to achieve the goals agreed in their NDIS plans. The role focuses on addressing challenges, assisting participants to connect with supports and build capacity and resilience. These two exciting roles are being offered as full time positions.

To be successful in this role you would meet the following criteria

  • Minimum relevant qualification with 3 or more years’ experience in the disability/mental health/community services sector
  • Significant knowledge of NDIS funded plans with prior experience in a support coordination role or similar
  • Demonstrated experience in case management and supporting people with complex needs
  • Connections and networks of local social and community services available to people with NDIS funding
  • Thrive in a supportive team environment
  • Clear police, working with children & DWES checks plus a valid driver's licence & the right to work in Australia
